When I began my nursing studies several years ago I only studied in and familiar with traditional classroom. Now as a nursing student, because of my busy schedule, I embraced online learning to pursue my further studies in nursing. Here by I would like to explain the bed rail entrapment program implemented in my work place. Several surveys of deaths occurring in the bed environment demonstrate the risk of entrapment when a resident slips between the mattress and bed rail or when the resident becomes entrapped in the bed rail itself. At risk category in elderly are on agitation, delirium, confusion, pain, uncontrolled body movement, hypoxia, fecal impaction, and acute urinary retention that cause them to move about the bed or try to exit from the bed. The absence of: timely toileting, position change, and frequent individualized care are factors that may also contribute to the risk of entrapment. The risk may also increase due to technical issues such as improperly sized mattresses, bed rails with winged edges, loose bed rails, or design elements such as wide spaces between vertical bars in the rails themselves.

Community of practice members is voluntary, part-time whereas in team, members are selected and work full-time.
Electronic health record (EHR) and electronic medical record (EMR) sounds similar and used interchangeably, there is a difference between them. Electronic medical record is collection of medical history, diagnosis and treatment of a patient in one practice. To communicate between health professionals, EMR needs to be printed and sent by person or mail to the specialist. They are designed to share and access health information about patient between health organizations and professionals in real-time. Patients can also access their health records in EHR but depends upon the jurisdiction law.
When all health care providers can have same and current information, it can provide better care outcome for patients. All these discussion helped us be learn about social media policies, be sensitive about patient’s privacy and confidentiality, and to maintain ethical practice even on social media post. We also discussed about patients’ communities of practice vs. team, advantages and disadvantages of team care for patient. Even though social media may have negative effects such as breach in privacy and confidentiality, mistrust in health care professionals, loss of job/career, etc.
, it has also positive aspect if used properly. Social media can be a great platform for patients and health professionals to discuss on health topics which may provide emotional support, sense of connectivity/belonging, learning ground, provides a sense of community, medium to enhance organization and planning, etc. In the summary, social media policy and course should be mandatory in nursing school and practice setting; nurse should have strong knowledge of law, CNO social media policies, organizational policy on social media use, ethical practice standards, and be accountable for unprofessional social media post.Nurses have an ethical responsibility to safeguard information obtained in the context of the nurse-client relationship. When clients entrust their health care and health information to a nurse, they expect and rely on it being kept confidential.
